Data Subject Application Form

Under Article 11 of the Turkish Personal Data Protection Law No. 6698 (the "KVKK"), data subjects (the "Applicant") are granted the following rights:

• to learn whether your personal data is processed
• to request information if your personal data has been processed
• to learn the purpose of processing and whether the data is used in accordance with that purpose
• to know the third parties, in Türkiye or abroad, to whom your personal data is transferred
• to request rectification if your personal data is processed incompletely or inaccurately, and to request that this be notified to third parties to whom the data has been transferred
• to request erasure or destruction of your personal data where the grounds requiring its processing have ceased to exist, even though it was processed in accordance with the KVKK and other applicable legislation, and to request that this be notified to third parties to whom the data has been transferred
• to object to a result to your detriment arising from analysis of your data exclusively by automated systems
• to claim compensation for damage arising from unlawful processing of your personal data

Under Article 13/1 of the KVKK and the Communiqué on the Procedures and Principles of Application to the Data Controller, applications relating to these rights must be submitted to our Company in writing, or by sending an email to [email protected] from an email address you have previously notified to us and which is registered in our systems, or by other methods to be determined by the Personal Data Protection Board (the "Board").

Applications made to our Company "in writing" may be submitted by printing out this form and:

• applying to our Company in person
• sending it through a notary or by registered mail with return receipt
• where the Applicant has an email address registered in our systems, sending it from that address to our [email protected] address

Under Article 13/2 of the KVKK, your application will be answered free of charge within thirty days of reaching us, depending on the nature of the request. Where the process incurs an additional cost, the fee set out in the tariff determined by the Personal Data Protection Board may be charged.

If your application is refused, if you find the response insufficient, or if it is not answered within the period (30 days), you retain the right to lodge a complaint with the Personal Data Protection Board under Article 14 of the KVKK.

If your request concerns a particular conversation, stating the date and time of the call and the number called or calling will help us locate the record. For requests relating to call records, in line with the settled practice of the Board, the transcript of the conversation is provided rather than the recording itself, and personal data of third parties appearing in the transcript is masked.

In order to eliminate legal risks that may arise from unlawful and improper data sharing, and in particular to ensure the security of your personal data, our Company reserves the right to request additional documents and information (a copy of an identity card or driving licence, etc.) to establish identity and authority.

If the information you submit within this Application Form is inaccurate or out of date, or if an unauthorised application is made, our Company accepts no liability for claims arising from that incorrect information or unauthorised application.
